Dr. Danziger of Southgate ENT is a board-certified otolaryngologist treating conditions of the ear, nose and throat (ENT). Southgate ENT is an inclusive practice open to patients of all ages from newborns to adults.
Dr. Danziger
Dr. Danziger obtained her medical degree from the State University of New York at Buffalo. She completed a general surgery residency at Beth Israel Medical Center and an otolaryngology residency at the State University of New York at Buffalo. She is board-certified in otolaryngology.
New Patients
To schedule an appointment, simply call our office at (716) 712-0855. Please have your insurance information readily available. We will also ask for your pharmacy information so that any prescriptions can be electronically submitted. Please bring your current insurance card, co-pay and complete list of medications to each appointment.
The day of your appointment, we will ask you to complete a health history questionnaire. You may complete this prior to your appointment by printing out the form.
We regret that we cannot accommodate walk-in visits or provide life-threatening emergency care in our office.
We require advanced notification if you need to cancel or reschedule an appointment.
Insurance
Dr. Danziger does participate with most major insurance companies and self pay patients. Patients should be aware of their insurance coverage, authorization requirements and network providers.
It is our patient’s responsibility to notify our office in advance of any insurance changes.
Prior Authorization
Some medical testing(ex. CT, MRI) or procedures(ex. surgeries) require prior authorization from your insurance company. In the event that you are affected by this policy, our office will initiate the authorization process for you.
High Deductible Plans
Please note that this is an agreement you made with your insurance carrier. You will be billed all applicable fees as determined by your insurance carrier.
Self Pay Patients
Please be aware that all payments are due at the time of service.
Policies
- If your insurance requires a referral, it must be in place at the time of your appointment in order for you to be seen. Please contact your primary care physician for a referral.
- Copays are due at the time of your appointment. There will be an additional fee for billing of your co-pay.
- We do charge a $25.00 fee for an appointment missed without notice of cancellation.
- We accept most major insurances.
- We accept Visa, Mastercard, Discover, cash or check for payment. We do not currently accept American Express.
